This link took me to the sign in page of Survey Monkey and when signed in it did not take me to your survey. You have to click the "send survey" button on the top right of your survey, which opens a page with a "survey web link". You can then copy and paste that link onto your blog page. You don't copy and pastes the URL from the address bar. I hope this helps!
